The whole of the story has really been about the Smiling Man and his wife's death. It's been Smiley's whole driving force to get "revenge" on Ringo. Nothing would ever get resolved unless THAT was resolved–one way or the other.

In that fourth panel, there are some cameos. I thought it might make sense, as the realities started to merge together, to show some almost about to break through. The semi-humanoid moth-like alien is Fructose Riboflavin, a villain from THE INEXPLICABLE ADVENTURES OF BOB—since it's a black-and-white comic, I had to guess at the color, and made him green. The older guy with glasses and a goatee is Dr. Catastrophe of EVIL OVERLORDS UNITED–the EOU being the main bad guys in the Crossover Wars, the largest webcomics crossover ever. The red-demon-thing is a Q'Arth demon from MAGELLAN—their invasion of Magellan's reality nearly destroyed Magellan's Earth and was the impetus for Magellan being formed. The creepy thing in the dark with the catlike eyes is just a creepy thing in the dark with catlike eyes. The final image there is of Nailbat, from ANTIBUNNY, a vigilante rabbit with a bat with a nail embedded in it—which is a pretty scary thought in and of itself.
We'll find out next time why none of the previous readers/keepers of Rachel's journal figured out what effect the journal entry's would have on Smiley, until the Green Avenger discovered it. Plus the final fate of Smiley. We're getting VERY near the end, people.
