They are speaking in thought speak. Saturno does understand them. Saturno is telling the story so Saturno would not include what was said if she/he can't "hear" it.

It's regression. Losing parts of his/her body has resulted in Saturno turning back into forms it was before. Thus a bird. So I guess it would be more subconcious/reflexive.
