The comic didn't stick around long enough for this to become clear but in the Malcontents universe the a unicorn sheds it horns occasionally like some animals with antlers do. I got the idea from a children's book in which the King commisions his wizard to capture a unicorn horn. I wish I could remember the name, it was and still is a classic as far as I am concerned.

So in short while it no doubt hurt a long (as you can see Joy is in shock) to remove it violently it would be possible with normal human strength if the horn was already getting ready to shed as was the case here. There was going to be a humorous scene in future where P#6 not fully aware of these facts, tries to remove a growning horn from Joy a second time to much comical disaster.
