Actually, it's a simile.

In classical art, Death is often seen bearing a scythe in one hand, and an hourglass in another. I want to play around with the aspects of each of them in my own way. I may bring about the scythe later, but in case I don't, pretend that the back of his hood is the scythe.

I don't think you can see it, but I was playing with a sort of visualization regarding Death's formation of the "sand" into the soul gem. I'd played with a lot of different ways he sees their lifespan - an hourglass over their head, seeing the individuals themselves like a lifespan, or - as he's mentioned before - instinct. He just looks at a person, and knows if their time is limited.

And when he looks into your eyes, you get a cold chill as he sees every intimate detail of your life… past, present, future.

I think the List probably already has the information regarding deaths, but he doesn't usually care to use it. It's easier just to sniff them out.
