Dr Johanna Watson is telling the tale. The script is based on the beginning of A Study in Scarlet (A few paragraphs of Watson's story compressed down).
I think she looks rather ghoulish in the first few pages, but prettily dressed in an 1871 afternoon dress, just the same.
