He's using both the hat and the pendant, he can indeed do that with the spikes but without using the pendant, they wouldn't have had good aim ^_^
He is completely in Zeno's head, so yes he can be harmed. Think of it as similar to Nightmare on Elm Street (I've still not seen that movie entirely though)he can't fix his oen injuries here either, since it's really him, not an illusion.
